Community Occupational Therapist / Physiotherapist - Care Home Team
Job Overview
Important note for candidates - we are unable to offer sponsorship for this role. This is a 12-month fixed term contract. Secondment would be considered for an internal applicant.
Position is a full time post, however, we would consider part-time working hours and a flexible working approach.

We are seeking a motivated and enthusiastic Occupational Therapist or Physiotherapist to join the Leicester, Leicestershire & Rutland (LLR) Care Home Team. The successful candidate will work in line with the Enhanced Health in Care Home (EHCH) principles, taking a pro-active approach to workforce development and early identification of escalation of need to support ageing well and quality of life for individuals in a care home setting.
Utilising a ‘train the trainer’ approach with care home staff, you will facilitate Falls prevention training and Positioning education around body shape changes. A variety of educational methods will be adopted, to include floor walking, on the job learning, formal & informal presentations, Q&A sessions and development of a community of practice for care home staff.
Working with a reduced caseload, you will ensure early identification, assessment & intervention of care home residents for therapy input. This will include working in partnership with Care Home Team Leads, Therapy Teams and other key stakeholders.
You will provide clinical leadership within therapy hubs, across the service line, to promote proactive clinical & educational interventions with care homes.
The post holder will work autonomously to analyse data to determine targeted training and clinical interventions. Reporting and monthly evaluation of interventions to provide data and outcome measures for system wide sharing.
You will have the clinical expertise and communication skills to be able to identify areas of good practice and to effectively challenge areas for improvement as part of training interventions. This will include collating data to identify trends and themes for further evaluation with Care Home Team leads.
It is essential that you hold a valid full driving licence and have access to a vehicle that can be used for work purposes.

Job Description Summary (Main duties and responsibilities)
· To promote the independence and well-being of older people and vulnerable adults in Care Home settings.
· To provide clinical oversight and advice around appropriate escalation and referral to Leicestershire Partnership Trust (LPT) services for identified individuals, to enable timely input and prevention of harm.
· To work autonomously with a solution focused approach, utilising hard / soft data and resources available across the system to identify and prioritise the care homes for intervention.
· Create database to record training interventions and to identify falls and positioning champions within the care homes.
· To liaise with key health and social care stakeholders, using data analysis to prioritise care homes for targeted intervention, and provide feedback as required.
· To collate feedback following the training intervention and community of practice forums to support Care Home Team service development.
· To ensure training content is contemporaneous and in line with current standards and guidelines and update as required.
· To maintain professional development through engagement with clinical supervision and training opportunities.
