Hours/Shift 9:00 a.m. - 6:30pm. EST with every 3rd weekend rotation

Position Purpose:

Our Virtual Care Coordinator RNs (VCC) provides virtual, professional nursing care to the organization’s clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Nurse Practice Act, any applicable licensure/certification requirements, and the organization’s policies and procedures. The RN is fully responsible for the virtual assessment, care planning, telephonic intervention, and overall virtual care remote monitoring and follow up. Provides triage/on-call support to home health/hospice agencies.

What You Will Do:

  • Effectively and efficiently prioritizes virtual care/ telemonitoring alerts visits based on client condition, physician orders and available resources.

  • Maintains a current working knowledge of applicable Federal. State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.

  • Accurately assesses clients at admission to the virtual care center, at required time points, and as needed, utilizing virtual care equipment.

  • Informs the client/caregiver about the virtual care center, care/services available, client obligation for payment (if any), and other responsibilities and rights, before care is initiated.

  • With the physician, office case manager, and client, develops appropriate assessment/needs-based plan of treatment including attainable goals and agency services needed to improve and/or stabilize client’s medical condition and promote independence, and readmission rates under 5%. Participates in timely revision of the plan of treatment as appropriate.

  • Makes appropriate referrals for evaluation/care to other disciplines and services if need identified, and coordinates care with office care manager to ensure effective and efficient care is provided.

  • Utilizes interview, evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in formulating interventions and making recommendations to the physician, agency, and client.

  • Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and/or Case Manager/designee and obtains orders for changes in the plan of treatment to respond to the client’s condition.

  • Initiates and assures implementation of orders for medication/ treatments as prescribed by the physician in accordance with the plan of treatment.

  • Utilizes and implements appropriate tools and pathways per guidelines and/or policy and procedure (e.g., education checklists, pathway protocols, Home Program Instructions, phone visits etc.).

  • Participates in quality improvement processes including peer review as directed.

Minimum Qualifications:

  • BSN required by an approved Nursing education program.

  • Compact RN License Preferred

  • Two-three years' experience in home healthcare or comparable experience in acute care cardiac, ICU, CCU, or ER, triage med-surg or home health telemonitoring

  • Hospice and/or home care Experience Preferred

  • Epic or Home Care EMR Experience Preferred

  • Demonstrated ability to achieve hospital readmission rates <5%.

  • Well-developed communication and interpersonal skills to provide effective interface with professional and clinical staff, patients, and other related customers.

  • Demonstrated ability to effectively use health coaching and motivational interviewing techniques to assist patients in achieving health goals. Health Coach Certification a plus.

  • Current Registered Nurse licensure in the state of practice.

  • Must have current Driver’s license and reliable transportation to and from work site.
