Social Determinants of Health Community Strategist

arenaflex is seeking an experienced and driven Social Determinants of Health Community Strategist to join our team and make a positive impact on the health of our customers. Our ideal candidate should have a passion for addressing social determinants of health and a commitment to creating solutions that improve the well-being of individuals and communities.The Social Determinants of Health Community Strategist will provide leadership and strategic direction to identify, develop and sustain community partnerships that incorporate social determinants of health to improve health outcomes and reduce health disparities. This individual will be responsible for establishing and maintaining relationships with key external stakeholders, including hospitals, government agencies and community-based organizations.To be successful in this role, the candidate must have a strong background in public health and/or social determinants of health, along with the ability to develop and execute integrated and effective strategies and programs. The ideal candidate should also have excellent interpersonal, communication and organizational skills, as well as the ability to collaborate with a wide range of individuals and teams. Responsibilities: Develop and execute integrated and effective strategies and programs to improve health outcomes and reduce health disparities. Establish and maintain relationships with key external stakeholders, including hospitals, government agencies and community-based organizations. Identify and develop community partnerships that incorporate social determinants of health. Provide leadership and strategic direction to sustain community partnerships. Develop and implement programs that address social determinants of health. Collaborate with a wide range of individuals and teams. Analyze data to evaluate the impact of social determinants of health initiatives. Develop and share best practices for addressing social determinants of health.
